Carrier Analysis
Letter

A Carrier Analysis Letter is specifically created on behalf of the insurance carrier regarding a claimant’s medical treatment summary. The carrier uses the analysis letter to provide a summary of medical records to an independent medical evaluator such as a doctor performing a Designated Doctor Examination, Required Medical Examination and Peer Review.

Case Details Provided by Insurance Carrier

This section of the analysis letter will contain any specific details provided by the insurance carrier to ensure the recipient of the Carrier Analysis Letter is aware of vital information regarding the claimant's case.

Chronological Treatment Summary

Our Chronological Treatment Summary provides a thorough and detailed review of all office visits in chronological order. Each treating doctor visit will list the date of visit, doctor/clinic, subjective complaints, physical examination findings, doctor assessment and plan of care.

List of Medical Records

The records list will contain the treating providers name with each visit date, type of diagnostic study and dates, type of procedure and dates, physical therapy clinic name with visit date range and a list of any other important information reviewed.

Bookmarked Tabs in Electronic Record File

In addition to the medical record summary, you will receive a PDF file of medical records with indexed/bookmarked tabs that will allow you to navigate through the record file to locate specific medical notes.
