Medical Record Summary
A Medical Record Summary is used by independent medical evaluators, insurance carriers and attorneys to simplify the review of a patient's medical history. The MRS significantly reduces medical record review time while allowing you to focus on the relevant details of the case.
The MRS includes all the data concerning the patient’s injury and its management. The summary will contain a concise description of the accident and immediate care. It will also provide details of the treatment provided and the results of diagnostic testing.
Our standard MRS template includes patient demographics, medical record review list, and a chronological treatment summary. In addition, an indexed medical record file allows you to navigate through the multiple pages of documentation for an easy to follow review.
Our MRS files are completed within a 48 hour turnaround time for most cases.

Chronological Treatment Summary
Our Chronological Treatment Summary provides a thorough and detailed review of all office visits in chronological order. Each treating doctor visit will list the date of visit, doctor/clinic, subjective complaints, physical examination findings, doctor assessment and plan of care.

Procedures, Diagnostic Testing, Therapy
Following the treatment summary, our medical records team will chronologically list procedure notes (surgery, injections, etc), diagnostic reports (MRI, CT Scan, x-ray, etc) and physical therapy visits.

List of Medical Records
The records list will contain the treating providers name with each visit date, type of diagnostic study and dates, type of procedure and dates, physical therapy clinic name with visit date range and a list of any other important information reviewed.

Bookmarked Tabs in Electronic Record File
In addition to the medical record summary, you will receive a PDF file of medical records with indexed/bookmarked tabs that will allow you to navigate through the record file to locate specific medical notes.
