Simply locate the note you are looking for by clicking on the medical record tab listed in the Table of Contents.

The bookmarked PDF file of medical records will contain a Table of Contents with tabbed records for each office visit, procedure note, physical therapy record, diagnostic report and more.

Each office visit is identified with the date of the visit and provider name, type of procedure, or type of diagnostic study.

Avoid unnecessary pages such as fax cover sheets, blank pages, etc. The bookmarked file will separate miscellaneous pages for your convenience.
